Farewell To Falls

Farewell to Falls is a free, individual, home-based program for older adults living in Santa Clara or San Mateo Counties.

​

An occupational therapist provides two visits to look at strength and balance, home safety, medication review, and daily living activities. Please call 650-724-9369 for more information.
